CONSENT FOR PARTICIPATION IN ACTIVITIES & EXCURSIONS

I give permission for my child to participate in all planned and supervised activities and off-site excursions arranged by R3 Voyage Kids Club, including but not limited to:

●    Ice skating

●    Swimming

●    Boxing and martial arts

●    Water play or splash parks

●    Climbing, obstacle courses, or inflatables

●    Theme parks, farms, and museums

I understand that these activities may carry an inherent risk of injury despite all reasonable precautions being taken. I confirm that:

●    I have disclosed all known medical conditions, allergies, or additional needs that may affect my child's participation.

●    My child is physically fit and capable of participating in these activities.

●    I will ensure they have appropriate clothing, swimwear, and footwear where required.

LIMITATION OF LIABILITY

By booking this class, I acknowledge and agree that:

●    R3 Voyage Kids Club, its staff, volunteers, and affiliates accept no responsibility for personal injury, accidents, or loss of property except in cases of proven negligence.

●    I waive, release, and discharge the Club from any and all liability arising from injury or illness incurred during participation, travel to/from excursions, or while using equipment provided.

●    I understand that staff will act in accordance with professional duty of care and safety protocols at all times.

●    I understand that R3 Voyage Kids Club reserves the right to cancel a child’s participation in an activity if safety or behaviour becomes a concern.

EMERGENCY TREATMENT

In the event of an emergency, I give permission for the staff of R3 Voyage Kids Club to:

●    Administer first aid

●    Seek medical attention

●    Transport my child to the hospital via ambulance if deemed necessary
